By attending an appointment at Pink Pineapple Beauty Co., I am confirming that I recognize that with any treatment, use of facilities, and/or use of equipment, there are risks involved. I acknowledge and agree that I am responsible for my own health; that the spa associates and/or technicians are not health care providers and cannot treat or diagnose any medical conditions. I understand that I am responsible for disclosing/discussing any questions that I have concerning my health conditions (if any) prior to any treatment or services rendered, and should health related symptoms occur, I will alert spa staff and cease participation of treatment immediately. In the event that I have reason to believe that medical clearance is needed prior to obtaining a spa service, I agree to first consult with a physician and obtain written permission prior to obtaining any service. By voluntarily choosing to receive spa related treatments and/or participating in spa related activities, I warrant that to the best of my knowledge I have no impairment that would prevent me from receiving such treatments and/or engaging in such participation.
By participating as a model and receiving reduced prices, I agree to allow Pink Pineapple Beauty Co. to share photos or videos of my service across social media platforms for marketing purposes. Non-model clients will indicate their choice to participate in a photo release below. Consequently, I hereby release the spa, and its parent corporation(s), subsidiaries, affiliated corporations, and their respective officers, directors, employees, and waive any and all claims, liabilities, or damages for personal injuries that I may experience directly or indirectly from receiving spa related treatments, utilizing the spa facilities.
Refund policy:
Services received cannot be refunded.We will gladly adjust any service you receive within Three days of your service. If you are not satisfied with the work performed, we require that you return to the spa so that work can be visually inspected. At that time, we will make every effort to correct any problems to your satisfaction by rescheduling you with the service provider who performed the initial service, at no additional charge to you.
Cancellation policy:
Cancellations made 24 hours or less before an appointment will be subject to a charge of 50% of the services booked. Cancellations made more than 24 hours in advance will be processed without a fee.
